Consent form for patient text message
By signing the text consent form you are agreeing to the practice contacting you by SMS re. results, appointments, appointment reminders, practice information etc.
GDPR consent form
By signing this form you give consent to allow another individual to collect your medical data from Rathdrum Medical Practice and you are nominating next of kin who you are agreeing can be contacted if necessary by the surgery on your behalf
