To register with Rathdrum Medical Practice, you'll need to complete our new patient registration form below or collect one from reception. Applications to join the practice will only be accepted on the first working day of the month, as we do not have the resources to review outside this time. If the first of the month Applications will be accepted depending on  availability and practice capacity and will vary monthly. This is to ensure patient safety and maintain a high standard of care, to our currently registered patients.

Important: For GDPR reasons, we cannot accept online submissions or email of the completed form. Please post your completed form to us, or drop it in by hand at the surgery.

What Happens Next?

We do not run a waiting list and you will be contacted when a slot/slots become free.
